Patient Registration
To make an appointment, please call our New Patient Registration department at (561) 366-4100 between the hours of 9:00 A.M. and 4:30 P.M. Monday through Friday. You will be asked to provide your insurance information and your referring physician’s name and phone number. Your medical records will need to be received prior to your appointment.
You may also print our patient questionnaire and fax it to (561) 366-4115. A PBCI representative will call you within 24 hours.
It is extremely important that our physicians have all necessary information to ensure a complete evaluation.
What to Bring to Your First Appointment:
- Radiology films (x-ray, CT / PET scans, MRI, etc.)
- All medications in original bottles
- Insurance cards
- Photo ID / Drivers License
